I enclose a copy of my 2001 Tax Return, together with all relevant supporting information.
In particular I enclose details of any PAYE underpayment brought forward at 5 April 2000, and any underpayment being adjusted in my Tax Code during the years ended 5 April 2001 and 5 April 2002 including all PAYE coding notices.
I accept that Fowles Spurling Cannon will not be responsible for incorrect calculations if I do not provide full and correct details, and that you will calculate my liability solely on the information I provide without any check of the accuracy of that information.
I enclose a cheque for £40 payable to Fowles Spurling Cannon in respect of your charges (including £5.96 VAT).
Please calculate the my Tax Liability for the year, and sent me a summary of the calculation.
I understand that Fowles Spurling Cannon reserve the right not to process this request, and to ask for further information or a revised fee in certain circumstances. Fowles Spurling Cannon will agree any revised fee with me before proceeding and will return my payment if this request in not processed within 21 days.
